About Sub-Acute Services in South 6 Ward at Monash Health

Monash Health is the largest public health service in Victoria, employing over 18,000 people across a range of specialities.

South 6 at Kingston Centre (Cheltenham) is a Sub-Acute Geriatric Evaluation Management (GEM) Ward providing care, rehabilitation or assistance with the recovery for a diverse range of patient who has been in hospital and requires a multidisciplinary input and intervention from clinicians of medical, nursing and allied health.

We are a very dynamic team dedicated in delivering a holistic approach to patient care in line with our Monash iCARE values and that all patients are treated with dignity and respect as they transition into a longer term Residential care or back into their home environment with community services.

About The Role

We currently have Permanent Part Time vacancies with varying EFT per fortnight for experienced Registered Nurses to work through a rotating roster consisting of 7 days, all shifts, amongst our dedicated and diverse team.

The capacity to work to undertake complex and safe discharge planning, participating in managing complex medical issues associated with ageing, participating in carer training, in falls prevention strategies, safe administration of medication management, continence management/bladder retraining, pressure area prevention and complex wound management, delirium and dementia management, recognising and managing clinical deterioration or End of Life care, with the overall aim to see that all patients are treated with dignity and respect and are truly passionate about delivering high standard of evidence based nursing care to all patient.

About You

Understanding the importance of patient centred care, you are experienced within an acute/sub acute hospital ward and have genuine interest in working and interacting with our diverse patient who, at times, can demonstrate a challenging behaviour due to previous stroke, recent infection/trauma or history of dementia.  You are a committed RN who looks to support a wider team.

You have been developing your clinical skills having finished your RN Grad Year rotation, you are ready to take this next step in your career. You lead by example, thrive working in a dynamic, fast paced environment and are truly passionate being an RN. You are an advocate of Monash Health’s Strategic Guiding Principles 

Your critical thinking, willingness to learn and ability to work cohesively and share knowledge within a team will see you excel in this role whilst we provide you with guidance, support and training to learn the rhythm of rehabilitation.

What You Need:

  • Demonstrated experience preferably within subacute/rehabilitation 
  • Current AHPRA registration
  • Australian working rights
