About the Service

RALS provides a comprehensive specialist assessment and liaison service, to optimize the wellbeing of older people, people with disabilities and those requiring rehabilitation. The RALS assessment clinicians work in collaboration with the Geriatric Medicine Program across all sites supporting General Medicine and Specialty units.

Responsibilities include; early needs identification, triage, coordination, assessment and liaison with acute and sub-acute providers to facilitate the development of management plans and patient flow through all services within Monash Health.

About the Role

The RALS service is located across multiple sites including Monash Medical Centre Clayton, Dandenong and Casey Hospital. This position is primarily based at one site but on occasion, travel to other sites is a requirement of this role. Comprehensive assessments of the older person, individuals with disabilities & those requiring rehabilitation are completed within general medicine & adult specialist units, encompassing the Emergency Department & Acute Assessment Unit as indicated. The RALS Assessment Clinician is a member of the Rehabilitation & Aged Care Liaison Service (RALS)

This service will co-manage patients that are identified as requiring the expertise and skill of rehabilitation and aged care services. The RALS clinician will provide assistance to facilitate transition to Rehabilitation and Aged Care services.

About you

This role has been designed to back-fill the Specialty Medicine RALS clinician over 3 days per fortnight. The successful candidate will be required to utilise their own Nursing or Allied Health clinical experiences in assessing and managing patients flagged for Sub-Acute and/or Residential Care. Patient Flow responsibilities will include facilitating the transition of our patients to Rehabilitation and Aged Care services. The role requires a strong focus on team orientated practices and communication between treating units within the hospital.

This back-fill role will include assessing and managing patients within:

  • Specialty Medicine, including Haematology, Cardiac, Renal, Surgical Patients
  • Neurological and Neurosurgical Patients
  • Patients Under 65 Years of Age
  • Stroke

Experience in Sub-Acute and an understanding of the National Disability Insurance Scheme (NDIS) and Aged Care pathways will be highly desirable in selecting the preferred candidate.
