Community Rehabilitation

Community Rehabilitation offers client-centred, goal driven rehabilitation services to maximise a client's independence, health and well-being following acute illness or injury. Community Rehabilitation consists of large interdisciplinary teams that work with the client to achieve their rehabilitation goals and provide home and centre based therapy.

Job Summary

Full time- Ongoing position

We are looking for an enthusiastic, dynamic and skilled Occupational Therapist to work closely within an interdisciplinary team. The Grade 2 Occupational Therapist is responsible for working within the Community Rehabilitation program to provide best practice client care, achieve maximal operational effectiveness, develop organisational competencies and create strong partnerships within and outside of Monash Health. Drawing upon professional expertise in rehabilitation and community practice, the Community Rehabilitation Occupational Therapist provides assessment and intervention to clients in the centre as well as their own environment, for example, their home or local community. The Community Rehabilitation Occupational Therapist participates in quality improvement activities within the multidisciplinary team and larger Occupational Therapy department. You will receive regular supervision from a Senior Clinician Occupational Therapist and have the opportunity to participate in and receive regular professional development. You will also be supported by the Team Leader and Manager. 

This position may be required to work at any of the Monash Health sites and may be centre based or home based.

What You Need:

  • Bachelor of Occupational Therapy (or equivalent qualification),
  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) – Occupational Therapy Board of Australia (practicing, no restrictions)
  • Current drivers licence
  • Experience in Community Rehabilitation/Hospital based Rehabilitation
  • Experience in Neurological Rehabilitation 
  • Experience in Public Health (desirable)
  • Experience in pre-planning reports for NDIS participants (desirable)
