About our GEM @ Home Program at Monash Health

Monash Health is the largest public health service in Victoria, employing over 18,000 people across a range of specialities.

GEM (Geriatric Evaluation and Management) @ Home provides a holistic and individualised approach to patients who are suitable for acute home-based nursing care. All patient care provided aims to meet the needs of the patient, the family/carer and the directives of the treating medical unit. Gem @ Home patients are in-patients of the hospital who reside at home in an invisible ward. Our patients can be offered this option by treating staff if the care type can be delivered safely at home.  Most patients prefer to be treated in their home, with their family or friends around them where they can resume normal activities and routines quickly.

About The Role

This is an excellent opportunity to join the Nursing team within GEM @ Home, which are currently experiencing a period of growth, as we look for experienced Registered Nurses to join the team. 

The case mix of patients includes patients assessed as requiring Geriatric Management and Evaluation care that can be provided in the Home.

We currently have multiple opportunities for experienced Grade 4 and Grade 3 Registered Nurses in an ongoing capacity. Both Full Time and Part Time applications will be considered, the roles are rotating in nature from Monday through to Sunday with the hours you will be expected to work are between 8am – 4.30pm.

About You

As an experienced Nurse, you have been developing your clinical skills and are ready to take the next step in your Career.

You will have demonstrated experience in an Acute setting with a background in GEM/Rehabilitation and/or the Community setting highly desirable.

With your excellent communication skills and caring manner, you will be happy travelling to the patient’s home environment within the community to provide outstanding patient centred care. With this, a Full driver’s licence is essential for this role.

You will have a natural passion for delivering a high standard of care and understand the need to enhance the patients GEM @ Home experience as you work towards their Rehabilitation goals. You will demonstrate a high level of Clinical knowledge and be comfortable working autonomously whilst being part of the larger team.

What you need:

  • Substantial demonstrated clinical Nursing experience, essential
  • Post-graduate studies, desirable
  • Full Drivers License – essential
  • Current AHPRA registration
  • Australian working rights
