About Us

MonashWatch is a new service which aims to keep some of our most vulnerable patients, especially those with complex chronic health issues, healthier at home. In so doing, we aim to reduce avoidable hospitalisations and Emergency Department presentations. MonashWatch will regularly call patients to conduct self-rated health checks, when a threat to a patient’s homeostasis occurs the Health Coach will be alerted to investigate and intervene. Based on our success to date, the service is expanding to cover a larger cohort of patients. The MonashWatch service is being piloted in Dandenong to the end of the financial year. The position has the possibility of extension pending pilot evaluation and delivery outcomes. 

About the Role

We are looking for a dynamic and experienced Registered Nurse or Allied Health professional to join our energetic and innovative team. The successful candidates will be adaptive, have exceptional communication skills, be culturally sensitive and enjoy working within a small multidisciplinary team. As this is a pilot, you will play an important role in service refinement and trouble shooting. The patients are diverse and often complex, there will be a strong focus on service development and patient centred care.  This role will provide an excellent opportunity for personal and professional growth.

The Health Coach helps manage risks associated with avoidable hospitalisations. The Health Coach builds trust and relationships, bridges boundaries between services so they appear to be integrated to the patients. They support clinical management within scope of practice, promote self-management and support their patients to navigate relevant services. The Health Coach works effectively and collaboratively with other service providers including the patient’s GP.

If you are a suitably qualified and experienced clinician who is patient centred and enjoys direct patient contact, both face to face and on the phone, then this is the role for you. You will be able to use your aptitude for supporting chronic, complex and aged related health issues with patients and providers, and enjoy enhancing the patient experience.

Selection Criteria Experience with working with older people and/or vulnerable people with complex chronic healthcare needs

Minimum of 4 years clinical experience and suitable for registration with the relevant professional body

Capacity to build and sustain a positive and productive inter-professional relationship to improve patient care and experience. Please provide an example.

Ability to ensure quality and effective care delivery is maintained in the face of adjusting priorities. Please provide an example.
