Transition Care Program(TCP-South 1)is offering a fantastic opportunity for a HAN to join our friendly team. This is a part time permanent position.

About South 1/Transition Care Program:

The Transition Care Program is a time limited, goal oriented, slow stream rehabilitation program offered to aged clients through to Monash Health network. These clients are acute admissions, but require further time and support to complete their restorative process and access longer term arrangements.

South 1 offers 20 bed based TCP places and is located at Kingston Centre.

The Transition Care Program (TCP) is a jointly funded program, between the Department of Human Services(DHS) and The Department of Health and Aging(DoHA). The purpose of TCP is to bring together all modes of Interim Care under a single program structure.

About the Role:

The Role of the Health Assistant In Nursing(HAN) is to provide support to the Nursing team in delivery of safe and quality patient care. The HAN is required to work under the direction and supervision of a Registered nurses at all time, within their level of education and assessed competencies and also under the clearly defined role parameters.

The Main objectives of this role are:

  • To participate in delegated care delivery relating to patients activities of daily living, ensuring that the personal need and comfort of patients is maintained.
  • To assist in providing a safe patient environment and additional supervision ensuring patient safety is maintained.
  • To enhance patient recovery by assisting with patient bed positioning, transfers, ambulation and other care activities as outlined in the HAN core activity list within the care environment.
  • Within the confines of the HAN role, to provide general support to the ward health care team, ensuring effective and efficient delivery.

About You:

If you are committed to improving the patient experience and have trained or completed an Health Assistant Nursing (HLT 32512) this is an opportunity for you to work with the great team.

You have proven interpersonal and communication skills, including relationship building, cooperation and conflict resolution and work collaboratively with others to deliver outcomes. You have completed the units of the Certificate III in Health Services Assistance Qualification as listed in the position description.
