Social Work Services

Social Work across Monash Health is responsible for the delivery of a broad range of Social Work services for clients across the lifespan in the inpatient and outpatient setting.   

This role will be responsible for providing clinical Social Work services for clients who are attending the supportive care clinic and the High Risk Foot Procedure Clinic. Well developed knowledge of chronic disease management, adjustment to illness and grief and loss theories as well as a good understanding of the social determinants of health and how they impact are critical. The Social Worker will be required to undertake complex psychosocial assessments and develop comprehensive care plans which includes working closely with a range of community providers to ensure clients are adequately supported.  

About you

As an experienced Social Worker with experience in the health setting,  you will be working closely with the multidisciplinary team to support the delivery of patient care in clinics and in a community setting.
