About Monash Children's @ Home (MC@H) at Monash Health

MC@H provides in-home and community based nursing services for infants and children with ongoing complex medical needs through a variety of programs, including:

  • Hospital in the Home (HITH) is the provision of acute care to public hospital patient in the comfort of the child’s home or other suitable environment. HITH is an alternative for in-patient stay for medically stable children requiring daily/twice daily acute care interventions to complete their admission at home. Whilst at home, children are visited and cared for by a team of qualified MC@H nurses, medical and allied health staff who communicate regularly with the child's primary treating medical unit
  • Post Acute Care (PAC) services providing short-term support at home to children whose healthcare needs have increased following an acute hospital admission, inclusive of nursing, physiotherapy, dietician, speech therapy and occupational therapy services
  • In collaboration with the Royal Children’s Complex Care Hub, care for children who either require the:Complex Care model of support due to their chronic and most complex medical conditions
  • Home care Program to support home training of home support care workers
  • School Care Program to support children/adolescents who would not be able to attend school without specific medical interventions being performed during the school day
  • Medium risk COVID pathway, providing care for COVID positive children requiring more intensive monitoring and outreach than offered by the GP but not requiring hospitalisation.
  • The case mix of patients for all MC@H programs include conditions from a medical, surgical and oncology/haematology background for newborn infants, children and adolescents aged 0-18 years.

About The Role

We are seeking an experienced and enthusiastic Nurse Leader and Manager who is ready to take the next step in their clinical career to join in this job share with the current Nurse Manager Clinical Services Leader. The role will continue the growth of the programs provided to children and their families/careers by relentlessly pursuing excellence to deliver exceptional care, outstanding outcomes and positive experiences for all.

This is a Part time Ongoing Job Share role working 16 Hours per week.

About You

You are a dynamic and self-motivated Nurse Leader and Manager with recent acute children’s nursing practice with a passion for providing care in the home or other community environments.  While you are able to work independently, you excel in consulting and working collaboratively within a compassionate, dedicated, and vibrant multi-disciplinary team to deliver positive results. Furthermore you are easily identified as an excellent nursing role model.  Likewise you are relentless in your quest to drive safety for patients and your team.

What You Need

  • Proven post registration experience, with current continuum of practice inclusive of children’s nursing
  • Familiarity with HITH, PAC & Complex Care
  • Sound knoweldge & undersntading of working in home & community environments
  • Current Victorian Drivers Licence
  • Post-graduate studies in management or leadership

Selection Criteria Registered with Nursing and Midwifery Board of Australia via AHPRA

A minimum of five years post registration experience

Recent nursing management experience

Demonstration of appropriate competencies and skills

Current Victorian Drivers Licence

Post- graduate studies in management or leadership.

Post Graduate qualifications in Paediatric Nursing /Special Care Nursing / Neonatal Intensive Care Nursing /. Maternal and Child Health Nursing / Midwifery / Community Nursing

A minimum 5 year post registration experience, with current continuum of practice inclusive of children’s nursing

Familiarity with HITH, PAC, and Complex Care

Understanding of working in home and community environments
